Drop-off/Pick-up Reminders

· Supervise preschoolers.

· Do not hold up lines.

· If you need to unload items from the trunk or visit with others, pull over and park.

· Do not pass cars in the drop-off and pick-up lines.

· Students should not cross the black top during drop-off/pick-up times.

· Students need to be escorted and cannot run on their own. 

 

 

SIGN AND RETURN THESE FORMS

 

Parent/Student Handbook Agreement Form. Complete and return the Agreement Form on page 1 of the handbook on Enrollment Day. Pictures of approved school clothing can be found in the handbook.

 

Permission for Medication Form. Bring completed form to Enrollment Day if your student/s need prescription, non-prescription, or over the counter drugs (e.g. cough drops) administered during school time. Medication cannot be administered in school without authorization. Complete and return form on Enrollment Day.

 

Asthma Consent Form. Students may self-administer medication for the treatment of asthma and anaphylaxis while at school. Complete and return form on Enrollment Day.

 

Free and Reduced Lunch Form. Your family might qualify for a free or reduced lunch rate. This program is funded by the government, which provides financial relief for those in need. Don’t hesitate to see if your family qualifies. Complete application and return to Mrs. Alexander on Enrollment Day.

 

Student Lunch Volunteer Form. Encourage your 5th or 6th grader to help Mrs. Malsbury prepare and serve lunch. Complete and return sign up form on Enrollment Day.

 

Internet Access Agreement Form. Complete and return form on Enrollment Day. This agreement is between the school, student, and parent. It grants students the right to access the internet for research, ensures parents that the school has done what’s necessary to protect the safety of your children, and ensures that students will not abuse their internet privileges. This form is required according to Archdiocesan technology policy.

 

Parent Volunteer Opportunities Form. Get involved! Sign up to help in a variety of areas at the school. Complete and return on Enrollment Day.

 

6th Grade Band sign-up Form. Led by Mrs. Frost, Music Teacher, School Band is for 6th graders and meets Mon., Wed., and Thur., 7:30am at St. John School. Students perform two concerts. Talk to your 6th grader about taking advantage of this wonderful musical opportunity.

 

 

ADDITIONAL INFORMATION

 

Student Information Card: Each student must have on file a Student Information Card. This card is crucial when your child becomes injured or  sick. In the event of such circumstances, parents receive a phone call and we need to know how to reach you during the daytime. New families must complete and return this card to the school office before the first day of school. Returning families need to update their contact information on Enrollment Day.

 

eStatements: Contact Mrs. Alexander if you would like to receive your monthly school statement via email.

 

Extended Care For K-6th: Wendy Robinson, Extended Care Director, can provide care for your Kindergartner-6th graders before school  from as early as 7am until school begins at 8:15am. Care is also available after school from dismissal until 5:30pm. Visit with Ms. Robinson on Enrollment Day to sign up or learn more about Extended Care and how we can help meet your needs. Extended Care is not available to preschoolers.

 

Hot/Cold Lunch Program: The school lunch program is designed to provide students with lunches that will meet their nutritional needs at a reasonable price. We are fortunate to have Mrs. Dorothy Malsbury as Director of the Lunch program. Mrs. Malsbury knows all about nutritional and healthy choices. She has exciting plans for the lunch program. Sign up your child/ren for the lunch program on Enrollment Day. The school encourages families to pre-pay their August 2008 lunches on Enrollment Day. The total cost for August student lunches is $10.50. Checks payable to “St. John School Food Service Program” and write in the memo line “August lunches.” Student lunches $2.10, Milk 50¢, and Adult lunches $3.50.

 

School Supplies: Families who ordered supplies through the school may pick them up on Enrollment Day.

 

Alleluia Singers: Led by Mrs. Frost, Music Teacher, Alleluia Singers is our school choir for grades 4th-6th that meets on Tuesday mornings at 7:30am. There is no cost to join, and it is a great experience for your child. Students participate in two concerts during the school year. Mrs. Frost will send home sign up information this fall. Encourage your student/s to participate.

 

Carpooling: Go Green! Contact Mrs. Alexander if you need help putting together a school carpool.

You will receive your school statement about the 5th of each month. Payment is due upon receipt. Send through school back pack mail. Pay by
